Homebound Services

  • Homebound services are provided for a student’s extended absence from school due to a severe, prolonged or chronic illness under a physician’s care or as an IEP placement.

  • Service is temporary with the purpose of keeping a student current with his/her classes.

  • Absences are anticipated to be a minimum of four weeks- shorter term absences are handled at the school level.

  • Requests are made by the student’s attending physician on a school referral form.

  • Forms are not backdated and are considered from the date of arrival in the Special Services office.
